How do I complete part two of the PIP claim form?

Using the details the DWP took from you over the phone during part one of the claim process, they will generate part two of the claim form and send it to you to complete at home. Part two of the claim form will include a barcode (to uniquely identify your form) and some parts of it will already be completed for you (for example, your name, address and date of birth).

Along with the form, they will send an information booklet on how to complete it and a freepost envelope in which to return your completed claim form. If you mentioned that you would like information in braille or audio, you should also receive a print copy of the claim form and information booklet.

PIP toolkit
